觉得应该这样:
219. If she _hadn't helped______ (not help) us that time, we weren’t able to work out this problem.
因为有"that time",说明是过去发生的事情,是对过去的虚拟,用过去完成时.

恩,忘了看主句,那就应该是:
If she hadn’t helped us that time, we wouldn’t have been able to work out this problem..
